Create new toolbar?

I need to find a source which will tell me how to :

1. Create a toolbar
2. Add buttons to toolbar
3. have output from buttons go to the current open document
4. Make it only visible in the html source view

Anyone have either part or all of this info?

no it's not - - I had an idea, but it entailed the items and I haven't found a site that shows how to do it - since I've never done it, I needed some help.
(btw - I'm way over school age)
